Please complete this form ONLY if you are having issues logging in to your Claimant Self Service Unemployment account AND would like to have your account reset.
The Department of Employment, Training and Rehabilitation will need this information to access your claim. Please make sure to complete the entire form and verify that all information is correct before submission. You will receive an email with your temporary password along with instructions on how to reset your password once you have logged in to their website: ui.nv.gov.
We ask that all other comments related to Unemployment be submitted via “Share Your Thoughts with the Governor”.
